First-time adult / lost or stolen

  • Form DS-11 unsigned until the clerk instructs you
  • Certified birth certificate, prior U.S. passport, or naturalization certificate
  • Valid photo ID plus a photocopy of the ID
  • One compliant 2×2 photo. photo rules
  • Payment for application ($130.00) and execution ($35.00)
  • Lost/stolen: add Form DS-64

Adult mail renewal

  • Form DS-82 if you still qualify
  • Your most recent U.S. passport book (to mail with the form)
  • One new 2×2 photo
  • Payment $130.00, no execution fee

Child under 16

  • DS-11 in person with the child present
  • Both parents/guardians with IDs, or DS-3053 plus notarized ID copy
  • Child citizenship evidence and proof of relationship
  • Child photo and fees $135.00

Passport card only

  • Same DS-11 path in person for first-time card applicants
  • Citizenship and ID documents as for a book application
  • Remember: card is not valid for international air travel

Lost or stolen replacement

  • Form DS-11 in person
  • Form DS-64 describing the loss
  • Citizenship evidence and photo ID as for a new application
  • Optional police report (does not replace DS-64)

Book and card together

  • DS-11 in person with both book and card application fees
  • One execution fee for both documents at a single appointment
  • Citizenship, ID, and photo requirements as for a book application

Common document mistakes

  • Signing DS-11 at home before the appointment
  • Hospital souvenir birth certificate instead of a certified copy
  • Wallet-size photo strip instead of a cropped 2×2
  • One combined check for application and execution to the wrong payee
  • Child application without both parents or a notarized DS-3053

Photocopy rules

Bring a photocopy of the photo ID you show the clerk, front and back on one page if the ID has info on both sides. Citizenship documents are originals; only the ID gets photocopied in the usual first-time adult pile.

Mailing a DS-82 renewal safely

Use a large envelope that fits the old passport without bending it. Track the shipment. Send payment to the address on the current form. Old addresses fail.

Rejected or returned applications

Fix only what the rejection letter asks for; do not restart with a second unrelated form. Double-check photo rules and where to apply before you resubmit.
